Patient Dialogue Inc. (PDI) strengthens caring and compassion in the delivery of quality and cost effective health care through navigation of the health care system, support for personal decision making, and introduction to, as well as coordination of, palliative services for patients and their families at the end stages of illness and end of life.
PDI is structured in the first generation of a start-up business to assist organizations to do a better job of helping patients and families who are facing end stage chronic illness and moving to end of life experiences to make critical decisions that serve to actively direct their care, not be a passive recipient. This is done though providing usable and understandable information, education, decision aids and coaching and consultation services from a Nurse Navigator.
PDI services help promote the needs of patients and families for dignity, respect and appropriate treatment while helping healthcare providers and payers increase quality of care, save money and achieve market differentiation.
